Care Partner Education

Providing education around the overall diagnosis of dementia, the different stages of dementia, communicating in manners that are understood during brain changes.

Services

Intake package

  • Introduction and Initial assessment – 60 mins
  • Individualized plan of care based on the overall assessment, interests and needs of the family as a whole.
  • Individual intervention and family education/support session – 60 mins
  • Follow up home visit – 60 mins

Respite Care

  • Providing 1:1 interventions with your family member at home or at leisure activity in the community.

Care Partner Education

  • Family sessions providing tools for approach and techniques to engage Seniors living at home with cognitive impairments and social behaviours.

Community Connection

  • Determining the services that meet your loved ones needs and scheduling necessary appointments.
  • Providing accompaniment to programs such as seniors’ day program, senior fitness programs, social activities and spiritual programs.
  • Interventions will support the client out in the community helping the client reach their social engagement goals.
  • Program goal is to have the client become independent in the community.

Dementia Support and Companionship

  • 1:1 therapeutic interventions in the home using positive approached to care strategies.
  • Some of the therapeutic recreation activities that can enhance your loved one’s cognitive function include: Sensory stimulation, Reminiscing, Cognitive exercises, Social engagement supported by past leisure history and Exercise programs.
